Responsibilities:
Conduct comprehensive needs assessments for individuals and families seeking support, identifying barriers and strengths. Develop and implement individualized service plans, connecting clients with appropriate community resources, such as housing, employment, healthcare, and mental health services. Provide ongoing case management, monitoring client progress, and adjusting service plans as needed. Offer counseling and supportive interventions to clients experiencing crises or facing significant life challenges. Advocate on behalf of clients with various agencies and service providers to ensure access to necessary support. Maintain accurate and confidential client records, documenting all services provided and progress made. Collaborate with a multidisciplinary team of professionals to coordinate care and support efforts. Stay informed about local, state, and federal resources and policies relevant to social services. Provide crisis intervention and de-escalation support when necessary. Participate in team meetings, case consultations, and professional development activities.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution. Current and valid LCSW or equivalent licensure in the state of practice. Minimum of 4 years of post-graduate experience in social work, with a focus on case management and direct client services. Proven experience in assessing client needs, developing service plans, and coordinating resources. Strong knowledge of social service systems, community resources, and relevant legislation. Excellent interpersonal, communication, and active listening skills. Demonstrated ability to work independently, manage a caseload effectively, and maintain professional boundaries. Proficiency in using electronic health records (EHR) or case management software. Empathetic, patient, and non-judgmental approach to client care. A reliable home office setup with secure and high-speed internet access is required for this remote position.
