We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing home health team. In this role, you’ll have the opportunity to deliver meaningful, one-on-one care that helps patients restore independence, improve function, and return to daily life with confidence.

What You’ll Do

  • Conduct comprehensive clinical evaluations to determine patient needs and therapy appropriateness
  • Develop, implement, and update individualized Plans of Care
  • Provide skilled OT interventions to improve:
    • Strength, coordination, and range of motion
    • Functional mobility and safety
    • ADL performance (self-care, dressing, bathing, eating, etc.)
  • Assess cognitive, sensory-motor, psychosocial, and environmental factors affecting function
  • Fabricate and maintain splints and adaptive equipment
  • Recommend home/environmental modifications to promote safety and independence
  • Participate in interdisciplinary care planning (IDG) to ensure coordinated patient outcomes
  • Maintain accurate, timely clinical documentation (evaluations, progress notes, orders)
  • Follow infection control and safety standards in all patient care activities

What We’re Looking For

  • Current Occupational Therapist (OT) license in good standing
  • Strong clinical judgment and patient assessment skills
  • Ability to work independently in a home health setting
  • Excellent communication and teamwork skills
  • Compassionate, patient-centered approach to care
  • Experience in home health is a plus, but not required

Requirements

  • Valid Occupational Therapy License

  • 1 year of occupational therapy experience preferred

  • CPR certification

  • Valid Driver’s License & Auto Insurance

  • HomeCare HomeBase experience preferred


Physical Demands

  • Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

  • Must be able to lift, carry, pull, or push up to 100 lbs..
