About the position
This remote Medical Assistant position at Woodlands Primary Healthcare supports Chronic Care Management (CCM) and Principal Care Management (PCM) programs. The role involves serving as a key contact for chronic disease patients, conducting monthly care coordination calls, documenting clinical interactions, updating care plans, and ensuring patient engagement with treatment goals between office visits. It requires a bilingual MA who can work independently from home with reliable internet and a quiet workspace.
Responsibilities
• Conduct monthly CCM/PCM outreach calls with enrolled patients to review symptoms, medications, and care plan progress
• Accurately document all encounters and time logs in eClinicalWorks (ECW)
• Coordinate care between specialists, PCP, and ancillary providers
• Assist with care plan creation and updates under provider supervision
• Identify and escalate urgent patient concerns to the clinical team
• Maintain compliance with CMS billing requirements for CCM/PCM time tracking
• Support patient enrollment and consent documentation
Requirements
• Minimum 2–3 years of experience in a clinical or care coordination role
• Proficiency in eClinicalWorks (ECW) — required, no exceptions
• Bilingual — English/Spanish required (spoken and written)
• Must reside in the Spring, TX or Katy, TX area
• Reliable home office setup with stable internet connection
• Strong communication, documentation, and time-management skills
• Ability to work independently with minimal supervision
Nice-to-haves
• Medical Assistant certification (CMA, RMA, or equivalent) preferred
• Experience with CCM, PCM, or RPM programs strongly preferred
