COMPANY: Athena Hospice of Rhode Island
LOCATION: We service the entire state of Rhode Island. This is an on-the-road position with mileage reimbursement. Candidate must be willing to travel.
PAY: $45 to $50 an hour, depending on experience. Weekly pay.
You plan, organize, and direct the hospice care plan for your assigned patients under the direction of the clinical director.
Experience & Education:
Must possess an active and unencumbered RN license in Rhode Island.
Must have a nursing degree from an accredited college, university, or school.
Prior experience in hospice is required.
Duties & Responsibilities:
Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.
Provides a complete physical assessment and history of current and previous illnesses.
Supervises hospice aides.
Provides professional nursing care by utilizing all elements of the nursing process.
Assesses and evaluates the patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care.
Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Counsels the patient and family in meeting nursing and related needs.
