About the Role

As part of our Chronic Care Management (CCM) outreach team, you’ll engage with eligible patients to book appointments with our nurses and GPs. Working primarily remotely, you’ll make outbound calls using our call centre systems, working with empathy, professionalism and a genuine desire to help improve patient health outcomes. We can entertain full-time and part-time opportunities!
